How we know Iran and North Korea won't use nuclear weapons

The main reason Korea and Iran wouldn't use their nuclear weapons is that they are not powerful or strong enough to survive the retaliation attacks by the numerous countries around the world who would subsequently invade them if they attacked somewhere without provocation. Them using their nuclear weapons would make it acceptable for their enemies like the US to attack them in the name of the 'war on terror' and their regimes and leaders would undoubtedly be killed and their countries leveled to the ground.
In the cases of these two countries in particular, the US has thousands of troops only a few hours away, in their huge bases in Iraq and Afghanistan in Iran's case, and South Korea in the case of North Korea, meaning an instantaneous land invasion could be implemented if they were to attack.
The leaders of these countries although crazed and backward, aren't stupid, and they know for all their threats and bluster, if they dared to attack anyone then they would certainly be invaded and deposed from power themselves, which isn't what they want to happen.
The nuclear technology that they do have is only on the level of the weapons the US and UK had in and just after world war 2, and so basically they are far out gunned anyway, their primitive technology not being too much of threat to the more civilized and powerful countries of the world. In fact with their technology being so far behind many of their intended targets, if they did use their weapons, then they might be unsuccessful anyway and would simply be detonated harmlessly into the sea, meaning they wouldn't actually damage their targets at all, leaving their targets free to attack them back straight away.
