Also try and have a desert, even if it's just a piece of fruit. Rice puddings, custards or yoghurts with fruit are all good choices.
I would also suggest taking a multi-vitamin & mineral as if you don't have adequate vitamins and minerals you may not be absorbing your food correctly. This could mean that all your hard work would fail and you still would not gain weight. If you still have not started making small gains after a couple of months then we need to look further.
Are you doing a lot of aerobic activity? It may be that you are not taking in enough carbohydrates for the activity you do. You have to decide whether you want to cut down on your activity or not. Whichever you decide, it is very important that you continue with some resistance training as this will help increase your lean muscle mass. If you decide to cut down on some of your aerobic activity then you should start to see results. If you decide to continue with your activity then we need to get you to take supplementation before exercising. You can buy a liquid glucose supplement which you can take just before or during (if you have long workouts like marathon and triathalon training) and these will provide instant energy without your body burning up your protein.
If all of this fails look to a bulk-up shake, there are lots of weight gain shakes in the body building section of your health food store. Some of the weight gain formula's will have high protein low, carbohydrates and no fat, other's will have high carbohydrates and low fat, the third type will have high carbohydrates and high fats this is the one you will need to take. Some brands for example Nutralife actually have pictures of the different body types that the shakes are designed for on their labels which make it easier to determine. We actually had to put my mother in law on these shakes to bulk her up and we just gave her two a day one for morning tea and one for afternoon tea and that combined with making her eat three proper meals did help the weight to go on.
In conclusion, increase your calories and make every mouthful a nutritious one and, hopefully, you'll be able to gradually make your way back into a health weight range.
