This is a question that could take literally days to answer.
In terms of recent films, I found certain scenes in Running Scared (dir - Wayne Kramer) to be quite viloent indeed. Also Kill Bill Vol One containes scenes of graphic violence reminicent of Hong Kong Cinema. Reservoir Dogs, Sin City, Gangs of New York, and The El Miriachi Trilogy are also some recent examples of violent films.
If you are prepared to look a bit farther afeild for your screen violence it may pay to look to some Italian Horror masters for depictions of violence on screen.
Director Ruggero Deodato's 1980 underground classic Cannibal Holocaust remains one of the most graphically violent films ever made. This essentially mondo film tells the story of a group of exploitative documentary makers who encounter the horror of canibal tribes in the South American jungle.
This film is definately not for the feint hearted. Wildy reviled and banned, it features graphic violence toward both humans and animals. Love it or hate it, it would definately have to be one of the most violent fims ever made.
Also in Italy, renowned horror director Lucio Fulci has a canon of quite violent fims under his belt. Some notable stundouts include Zombie, New York Ripper, and House by the Cemetary.
Japan is also quite fond of graphic depictions of violence, and writer/producer Satoru Ogora's Guinea Pig series of films. This series which includes such titles as The Flower of Flesh and Blood, The Devil's Experiment and He Never Dies are essentially excercises in what can be done with a twisted mind and some really talented make up effects people.
Rumour has it that Charlie Sheen was at a Hollywood party where a Guinea Pig film was showing and he was so incenced with what he saw that he contacted the FBI claiming it was a snuff film. This was of course false, but such a claim is a testament to the believeabilty and ferocious themes depiced in these films. Again definately not for the faint of heart.
As I said earlier, it could take days and much conjecture to answer this question and the aforementioned films ony really scratch the surface of screen violence.
I have compiled a list of other violent fims which may qualify for the title.
