The first thing to remember when packing your carry on is that the list of permissible items can change at any time. In the U.S., refer to the Transportation Security Administration's (TSA) website for an updated list. The current list is located on the following page:

Generally, common sense rules. If it can be used as a weapon or ignition source, pack it in your suitcase not your carry on. If it is a liquid or gel, make sure it is three ounces or less and packed in a clear bag. In fact, items packed in bags should be packed in clear baggies in order to make any inspection quick and painless. All prescription medications and clearly identified medical materials will be allowed. You need to organize these items together and inform both the checkpoint security and the security at the x-ray that you have such items. This will help expedite the process.
Having returned recently from a trip in Mexico, I can tell you that different countries may follow different guidelines, regardless of whether you are flying on an American-based airline or not. In fact, we found the Mexican regulations more severe. For example, in the U.S. you can now take bottled water you purchase after you go through security onto the plane. Not so in Mexico (although they donate unopened bottles to charity for you).
In a nutshell, be aware. If the Homeland Security Threat Alert is increased, then airport security will also increase. The TSA will also revise its permitted/prohibited list periodically. Check it before you head out.
