OK simple answer to a simple question. Basically When your brakes are new they work well because the pads which are usually a metallic and ceramic combination (metallic particles for durability and ceramic for stopping) have been worn down some of the signs are that your stopping distance steadily increases, you have pedal fade (it goes farther and farther to the floor), You can get grabbing where you vehicle pulls one way or another. This is caused by unequal braking distribution to all four wheels. and also a squeal from your brakes can happen what happens when this happens is you brake pads have worn down past the point when they should be serviced and a small metal prong is touching the brake disk or drum to let you know that they need to be serviced right away. When you notice that you are not slowing down as easily as before or some of the other signs and symptoms spring up that is when you should service you vehicles brakes.
