How to change the name on your passport

When your name changes, there are many documents that you must update. Don’t forget to update your name on your passport. Many think this may be a difficult task; however it is not as difficult as one might think. There are three scenarios for completing the name change on your passport.

Scenario 1: The Passport was issued less than 1 year ago

The simplest option is if your passport was issued less than one year ago. If you are able to legally document the name change, you can complete the process by mail. You will need to mail two items: your current passport and form DS-5504. The form DS-5504 is the Name Change, Data Correction, and Limited Passport Book application. You will also be required to submit evidence of your legal name change under state or federal law. This could either be a certified or original copy of your marriage certificate or other government-issue document. A recent, color photo may be required. There is only a fee for this service if you require it to be expedited. Use a traceable method of delivery when mailing your passport and application.

Scenario 2: The Passport was issued more than 1 year ago

If you are able to legally document your name change, this would be your option if your passport was issued more than one year ago. In this case, you will use form DS-82. This is the Application for Passport via Mail. Renewal fees will be applicable and can be found at http://travel.state.gov/passport/. When submitting your application, include your current, valid U.S. passport along with a recent color photograph. You will also be required to submit evidence of your legal name change under state or federal law. This could either be a certified or original copy of your marriage certificate or other government-issue document. If you are a minor, this is not an option for changing the name on your passport.

Scenario 3: You cannot legally document your name change or you are a minor under the age of 16 whose passport was issued more than one year ago

If either of these scenarios applies to you, you must complete your name change at a Passport Agency or Acceptance Facility in person. You will need to submit your current valid U.S. passport along with the completed form DS-11. This form is the application for a U.S. passport. The First Time fees will be applicable. You will also need to supply a minimum of three public records showing your date of birth and place of birth that also provide exclusive use of your assumed name for a minimum of five years.

When your name changes, it can be overwhelming to remember which documents need updating. However, changing your name on your passport does not need to cause any stress. It is a simple process as long as you know what category you fall in and what documents are required.
