How to lose your job ...

I'm going to focus on reasons other than a deliberate attempt to lose the job.
I would imagine the same reasons can be used if one wants to get fired for what ever reason.
Aside from all the normal things people do that cause a termination, such as too many sick days, lateness, taking too many breaks, or anything that requires common sense in keeping the job, there are some very valid reasons why you can be terminated without even doing the things I've mentioned.
You may almost be a model employee, but if you don't fit in, life can be rough on the job.
For instance, if you complain about how the boss runs the business to others constantly, it gets back to him and consequently, your out looking.
If you talk about others or show your dissatisfaction with whatever bothers you, and you become the chronic complainer, you could be expected to get those walking papers.
Failing to be a team player, and being a constant thorn in everyones eyes doesn't bode well for being accepted on the job, and if you are one of those that violate those rules that encompasses being on time, or absenteeism as well, then who's to blame.
