What is the Post Deployment Health Assessment for military personnel

The Post Deployment Heath Assessment is part of a program for post deployment that is required for service members or military personnel and Department of Defense civilian employees. Post deployment activities required are based on the type of deployment, a commander's decision, Department of Defense and Service policies any health threats that may or may not have been experienced during deployment. Activities include receiving various health assessments, medical follow-ups and risk communications.


Assessment Requirement


Individuals that return from various deployments may be required to complete a Post Deployment Health Assessment which includes completing DD Form 2796. This needs to be done before your redeployment date but not any earlier than the 30 day period before your expected redeployment date. The completed form will be included in your permanent medical record. If you are a reserve member you need to complete the form before being released from active duty.


Face-to-Face Heath Assessment


If you have completed a DD Form 2796 you will be required to complete a face to face health assessment, with service members that are being redeployed, from a professional or trained health care provider. This assessment needs to be completed within 30 days after you have returned home or have returned to a processing station. Your discussion with the health care provider will consist of our answers to the questions on the DD Form 2796. The discussion will also include mental health and psychosocial issues that are commonly experienced with deployments.


Medical Referrals and Follow Ups


When you have a health concern or have received a health referral you will meet with a trained health care professional or provider to evaluate your deployment related health issues. Reserve Component service members who have returned from their deployment and require a more in depth evaluation may be kept on active duty. You may remain on active duty until you are fit for duty or have been processed by the Disability Evaluation System.


Risk Communications


Risk communications consist of a health debriefing that is provided to personnel of the Department of Defense who are redeploying or have recently been redeployed. The debriefing will inform you of any health related medical, occupational, environmental exposures that you may have experienced. The debriefing will also cover and individual concerns you have and provide information about any medical follow-up that may be. Assistance with reintegration and adjustment to normal routine activities will also be provided.
